    New Consults

    My new client welcome packet consists of two forms-a Client Health Questionnaire (CHQ) and a Food Journal. The CHQ provides me with a complete health history and allows you to specify your symptoms, challenges and goals. The Food Journal allows you to record your current diet so that I can identify if your problems are food related. It also gives me a place to start when we transition you to foods that are healthier. The CHQ is an online form. Just click submit at the end. The Food Journal is a fillable PDF that you'll email back to me as an attachment.
